Position Summary:
The FBC Nurse provides direct nursing care, medication administration, and ongoing health monitoring for adults experiencing behavioral health or substance use crises in a 24/7 Facility‑Based Crisis (FBC) program. Working under the direction of the FBC Program Supervisor and within the NC Nurse Practice Act, the FBC Nurse implements provider-ordered treatment protocols, supports crisis stabilization and withdrawal monitoring, and promotes a safe, therapeutic environment. The FBC Nurse also assists with training and supporting facility staff in nursing-related tasks and clinical expectations.
Your Role in Our Mission:
*Clinical Assessment & Monitoring
- Perform nursing screenings, collect vital signs, complete withdrawal risk assessments (CIWA/COWS).
- Monitor medical, emotional, and psychiatric stability, including behavioral health safety concerns.
- Assess and document acute psychiatric symptoms, safety risks, and changes in condition.
- Participate in crisis stabilization interventions and support a safe, therapeutic environment.
*Medication Management
- Administer medication, including PRN and emergency medications.
- Monitor for medication effectiveness and adverse reactions.
- Ensure medical safety, accurate documentation, and controlled substance accountability.
- Implement provider-ordered treatment protocols, including detoxification and symptom-management orders.
- Consult with the provider as needed regarding patient status or treatment concerns.
Crisis Response & Team Support
- Lead de-escalation efforts and coach staff in trauma-informed crisis response.
- *Serve as the clinical liaison for the shift team, promoting consistent communication and care
- Provide clinical guidance to non-licensed personnel.
- Support the FBC Program Supervisor by reinforcing clinical expectations and standards.
*Documentation, Compliance & Quality
- Maintain accurate and timely documentation of screenings, assessments, interventions, incident reports, and medication administration.
- Participate in audits, quality improvement activities, drills, and incident reviews.
- Uphold infection control standards, patient rights, and emergency preparedness requirements.
- Comply with Easterseals PORT Health policies and procedures, accrediting body standards, and applicable regulatory requirements.
*Training & Development
- Assist the FBC Program Supervisor in training and supporting non-licensed personnel in medication‑related tasks, including vital sign collection, medication administration, medication safety, observation and reporting, and documentation accuracy.
- RN only: May validate task readiness within RN delegation rules.
- Complete required training within prescribed timeframes.
- Other duties as assigned.
What We’re Looking For:
- Active, unencumbered North Carolina RN or LPN license, or a valid Multistate/Compact RN or LPN license.
- One year of experience in behavioral health or substance use treatment preferred.
- Strong oral and written communication skills.
- Demonstrated sound clinical judgement within scope of practices; ability to maintain confidentiality and professional ethics at all times.
- Ability to manage multiple priorities and work effectively in a fast-paced crisis environment.
- Valid unrestricted North Carolina driver’s license, reliable transportation, and current vehicle insurance.
- Proficiency with EHR systems or technically savvy.
Equipment Used:
- Office & Communication Equipment: computer, printer, copier, phone, Microsoft Office, Electronic Health Record (EHR)
- Medical Equipment: Automated external defibrillator (AED), breathalyzer, thermometer, sphygmomanometer, pulse oximeter, stethoscope, centrifuge.
Essential physical requirements:
- Ability to stand, walk, or remain mobile for extended periods during a shift.
- Ability to lift, push, pull or carry up to 25-35 pounds occasionally (e.g., supplies, equipment, assisting patients).
- Ability to safely perform physical interventions such as supporting an unsteady patient, assisting with transfers or responding to emergencies.
- Ability to hear and respond to alarms, client needs and emergency communications.
- Ability to work in a fast-paced environment with potential exposure to individuals in crisis, unpredictable behaviors, and common clinical hazards (e.g. bodily fluids, sharps).
- Ability to use personal protective equipment (PPE) as required.
- Must possess the necessary abilities to perform tasks such as develop written communication as well as review work to determine accuracy.

Environment:
- Work is performed in a fast-paced 24/7 crisis-care setting serving adults with behavioral health and substance use needs.
- Frequent interaction with individuals experiencing acute psychiatric symptoms, withdrawal, or unpredictable behaviors.
- Exposure to clinical hazards such as bodily fluids and infectious conditions; PPE is required.

We are currently hiring for two schedules:
Night Shift: 7pm - 7am (Week 1: Wednesday, Thursday, Friday, Saturday; Week 2: Wednesday, Thursday, Friday) alternating between 36 hours one week and 48 hours the next, with rotating weekends.
Day shift : 7am - 7pm (Week 1: Wednesday, Thursday, Friday, Saturday; Week 2: Wednesday, Thursday, Friday) alternating between 36 hours one week and 48 hours the next, with rotating weekends.
We also offer a full benefits package for benefits-eligible positions.
Compensation & Benefits
- Competitive salary: $27-$30/hr. for LPN's and RN's: $32-$37/hr. for this full-time position
